Client Care Plan Report

This Report provides details of all Care Plans captured on a specified Client’s Profile in the Care Plan screen, including Client Contact Details, Next of Kin, etc. This report can only be run on an individual Client basis and cannot be run for all Clients.

Please Note: This Report does not include Activities added to a Client’s Care Plan from the Scheduling Wizard. Only Care Plans added individually to a Client’s Profile are included.

Search Parameters	Definition
Group	Allows you to search by Group
Client	Allows you to search by Client (you must select an individual Client)
Date From	Allows you to define a start date for the search period
Date To	Allows you to define an end date for the search period

Columns In Report	Data Displayed
Plan Period	Dates within which you have searched on the Care Plan Report
Name	Client’s Name
Client ID	Client’s ID Number
Address	Client’s Address
Phone	Client’s Phone Number
DOB	Client’s Date of Birth
Next of Kin (NOK)	Client’s Next of Kin
Start Date	Start date as captured in the Care Plan screen on the Client Profile
End Date	End date as captured in the Care Plan screen on the Client Profile
Time	The Start and End Time scheduled for the Care Plan (as captured in the Care Plan screen on the Client Profile)
Frequency	Frequency of the Care Plan (weekly, monthly, daily, etc)
Monday – Sunday	Days of the week this Care Plan relates to
Activities	All Activities that are captured in the Care Plan screen
