Invoice Template 5 – Individual Client Visits for Funded Services Only Regular/Sunday Hours

The Individual Client Visits for Funded Services Only Regular/Sunday Hours report provides a breakdown of Completed and Confirmed Tasks, Cancelled but Send to Billing and Payroll Tasks, and Cancelled but Send to Billing Tasks. The Report illustrates results separating for Regular Hours (Monday to Saturday) and Sunday Hours. All individual Tasks are illustrated, broken down by their respective Funding Subcategory.

Columns In Report	Data Displayed
Client Name	Client’s Name
Task ID	ID Number of the individual Task
Service Date	The Date the Task was scheduled to take place
Carer	Name of the Carer that attended the Task
Regular Hours (Monday to Saturday 00.00 – 23.59)
Hours	The Number of Confirmed Hours from Monday to Saturday.
Rate	Billing Rate as defined in the Schedule Billing Chart. In the event that a Billing Chart has multiple rates that will be calculated for individual tasks, the Task will appear on the report for each Rate and all individual Rates will be illustrated along with the number of hours and amount calculated against this specific Rate.
Billable Amount	Regular Hours Billable Amount for each Task based on the Confirmed Amount
Total Billable Amount	Total Billable Amount for all Tasks carried out during Regular Hours within the selected period
Total Hours	Total number of Completed and Confirmed Regular Hours for the selected period
Sunday Hours (Sunday 00.00 – 23.59)
Hours	Number of Confirmed Hours carried out on Sunday.
Rate	Billing Rate as defined in the Schedule Billing Chart. In the event that a Billing Chart has multiple rates that will be calculated for individual tasks, the Task will appear on the report for each Rate and all individual Rates will be illustrated along with the number of hours and amount calculated against this specific Rate.
Billable Amount	Sunday Hours Billable Amount for each Task based on the Confirmed Amount
Total Billable Amount	Total Billable Amount for all Tasks carried out on Sunday during the selected period
Total Hours	Total number of Completed and Confirmed Sunday Hours for the selected period
